Session 1: Intake Form, Preferences, and Goal Setting

In our first session, we’ll start by completing an intake form to get a clear understanding of your current health, lifestyle, and challenges. We’ll discuss your personal preferences, so your health plan feels realistic and enjoyable. Together, we’ll define your specific health goals and outcomes you want to achieve, setting a solid foundation for our work together.

Session 2: Delivery Plan & Grocery List

In this session, we’ll take the information gathered in the first session and create a personalized action plan tailored to your goals. This will include daily routines, meal plans, exercise recommendations, and specific steps to move forward. To help you stay on track, I’ll also provide a grocery list with healthy, nourishing foods that align with your goals.

Session 3: Follow-Up & Progress Check-Optional

During this follow-up session, we’ll review your progress, discuss any challenges, and celebrate your successes! I’ll offer additional guidance to keep you motivated, adjust the plan if needed, and continue supporting you in making sustainable changes for long-term health.

Together, we’ll move step-by-step toward your health and wellness goals with practical, tailored support. 

Further Sessions: To Be Discussed as Needed

Additional sessions will be scheduled based on your needs and goals. Whether you need ongoing support, motivation, or further adjustments to your plan, we’ll discuss what works best for you. Each session will be tailored to ensure you’re continually progressing toward a healthier, balanced life.

    Scoring & Interpretation

    • 0–10: Unlikely to be chronic fatigue, but mild stress or lifestyle factors may be contributing.
    • 11–20: Moderate signs; lifestyle and nutritional factors should be reviewed.
    • 21–30: Strong indication of fatigue; consider functional testing and holistic support.
    • 31–40: Very likely chronic fatigue or adrenal dysfunction; professional evaluation is recommended.
